Continuing Care Team

This is a new service provided by Leeds Community Mental Health Trust, which aims to provide continuing health care for children and their families. It is a team of nurses who work closely with the existing Community Childrens Nursing Team to provide additional support and nursing provision to children with continuing health care needs in Leeds.

Children needing assistance must be between the ages of 0-19 with continuing health care needs. They must be registered with a GP in a Leeds Primary Care Trust, or live within the Leeds boundary. They must also meet the Continuing Health Care Eligibility Criteria and have been assessed by the Case Manager for Children and be receiving a core service, for example from the Children's Community Nursing Team.
