Community Learning Disability Team – Leeds and York Partnership NHS Foundation Trust

The Community Learning Disability Team provides a specialist service to adults 18 and over with a learning disability and complex health needs.

How to use the service

The service has one point of referral with one form to fill in.

We take referrals from families, carers, other agencies and help with access to a range of services.

All referrals will go to the trust referral administrator who will pass it onto someone who can help.

The purpose of the team is to provide a service specifically for Service Users with profound and multiple learning disabilities whose complex health needs are associated with extremely high levels of dependence and their requirements for multiple and complex interventions to maintain their health status.

Within this team there is also the Day Services Nursing Team. This team of registered learning disability nurses are based in four Local Authority Day Centres in Horsforth, West Ardsley, Rothwell and Seacroft as well as Vine Further Education Centre. The team provides direct nursing care and liaison with other professionals and agencies.
