Reflections Community Support (18+ years old)

Reflections Community Support is a person centred organisation that provides outcome based services in the community. We enable and empower the people we support, to take charge of their lives and to become involved in decisions that affect their future and well being.

We provide a 24 hour support service for vulnerable adults to live independently in the Leeds area.

Who We Support

  • Individuals 18 +
  • Over 60’s at risk of social isolation.
  • Individuals with complex needs.
  • Mental health.
  • Learning disabilities.

What We Do

We provide a range of support servicies in the community to meet the needs of the people to make life changes and maintain their mental and physical well being.

  • We provide 1-2-1 person centred support.
  • Domiciliary care.
  • Day support in the community for individuals with complex needs.
  • Befriending/Volunteering.
  • Floating Support.
  • Support with telephone calls e.g. to health care professionals,key workers, advocates, benefits, housing issues, probation services and other agencies.
  • Support at CPA meetings, tribunals, contact meetings, court hearings and hospital appointments.
  • Support with maintaining links with family and attending social events.
  • Guidance and sign post the individuals to other agencies.
  • Support to access public transport.
  • Undertake domestic tasks e.g. support with laundry, cooking meals.
  • Support with budgeting, food shopping, planning and preparing healthy food.
  • Guidance and sign post the individuals to other agencies.
