This is a citywide multi-disciplinary team consisting of specialist social workers, registered community learning disability nurses and administrative support. The team provides a service for adults aged 18 and over, who have a learning disability identified as profound to mild, and who have additional complex social and health care needs.

The team undertakes the following:

• social and health needs assessments including identification of service provision for day, respite and residential care and also individual support packages
• direct support to enable adults to live independently in the community
• input relating to physical, emotional and mental health issues and behaviour management
• relationship work
• work around social and sexual health needs
• providing welfare rights information
• support to parents and carers.

They operate an open referral system enabling anyone to make a referral.
