This team works with adults who have a learning disability and who, because of their learning disability, have needs likely to require the involvement of a number of organisations for at least one of the following reasons:

• There is a significant risk to health and welfare.
• Without intervention the person would be unable to remain in or return to their own home.
• There are high levels of physical or mental dependency without appropriate support for the individual and/or their carer.
• Without support the carer would be unable to maintain their caring role.
• The service user is about to become a parent.
• A complex care package is required involving more than one organisation.

The Care Management Team does not work with people with multiple disabilities where the main impairment is sensory, physical or a mental illness combined with a mild learning disability. In these circumstances, they will advise on the best way of getting help.

The Care Management Team is a formal link between individuals and all the social and health services. They use a person-centred approach to planning, to try and make sure that services are appropriate to individual need.
