The Family Keyworker Team is the Social Services referral and assessment team for disabled children.

The team works with children and young people aged 0-13 years who have a complex health need and/or multiple disabilities. We offer short term and intensive input to enable families to manage and co-ordinate their various networks of care. The aim is to empower families and help them be as independent as possible by working flexibly around the child or young person's changing needs. We offer practical and emotional support to the child, young person and their family. The team consists of specialist workers. A referral can be made to us by parents/carers or professionals, but must always have the family's consent.
