Person Centred Planning

Person-centred planning is about listening to people and understanding what they want from their lives. It usually involves family, friends, advocates, professionals and services working together with the person at the centre of the plan.

People using person-centred planning are finding that it can:

There are different planning styles used, for example Essential Lifestyle Planning, PATH, Maps and Personal Futures Planning.

Leeds has set up a Task Group on person-centred planning to make sure it is happening and is making a difference to people's lives.
