Transition Plan

All children have to be educated up to the age of 16 years. Many choose to stay on at school, especially young people with learning disabilities. For example the special schools in Leeds educate children up to the age of 19 years.

If a child has had a Statement of Educational Needs (SEN) whilst at school, they are entitled to a Transitions Plan at the age of 14 (year 9) . This involves the young person, their carers and family, the Education Department and other relevant agencies such as Social Services (see Leeds Social Services) . The plan is used to decide what will happen after the young person leaves school, and will be reviewed every year until then.
