Young disabled people have the right to the same opportunities in education, training or employment as other young people. The Government recognises it is important to make sure that care and support continues as they move into adulthood.

Transition means change. An important transition for a young disabled person is from child to adult services. This can often coincide with a young person leaving school. Good preparation can make these changes easier. This age group is a priority for Person Centred Planning .
