The NHS have made a two-page Grab and Go guide for adults with learning disabilities and autistic adults.
The NHS designed the Grab and Go guide in partnership with people with learning disabilities, families and nurses.
The Grab and Go guide gives the information doctors and nurses will need if you go to hospital because of COVID-19.
The Grab and Go guide should be used as well as your Hospital Passport.
