Learning Disability Initial Intervention Team (LD.IIT)
From Monday 8th June, the new Learning Disability Initial Intervention Team (LD.IIT) will go live.
This 18-month citywide pilot will provide a consistent front-end response to Learning Disability referrals, focusing on timely screening, short-term intervention, reviews and signposting where appropriate.
The new team will consist of 10 Social Workers and Well-being Workers from existing teams across the Learning Disability service.
From 8th June, all Learning Disability referrals should be sent to LD.IIT.
· LD.IIT will screen all Learning Disability referrals.
· Queries about unallocated, passive, or pending cases should be directed to LD.IIT.
· Queries about allocated cases should continue to be directed to the allocated worker or relevant Team Manager.
Please use this pathway from 8th June and share it with relevant colleagues to support smooth and consistent implementation.
The team’s functions are as follows:
1. Screen referrals from the Contact Centre.
2. Manage safeguarding referrals for cases that are not open to an allocated worker.
3. Undertake short-term work, assessments, support planning, and reviews for unallocated cases.
4. Manage hospital discharges for unallocated cases.
5. Process COPDol renewal applications for unallocated cases.
6. Provide advice and signposting.
